In-office injectable and orofacial-pain therapies — therapeutic and cosmetic botulinum toxin, PDO threads, trigger-point injections, and care for TMD/TMJ, headaches, and chronic migraines.
Orofacial pain and migraine are deeply connected — tension in the jaw and facial muscles is one of the most common, and most overlooked, drivers of chronic headache. Specialty training in this area means treatment that addresses the actual source, not just the symptom. Bespoke doesn't rely on strict, one-size-fits-all dosing formulas; every treatment is tailored to your specific anatomy and needs.

Jaw pain, clenching, teeth grinding, and muscle tension in and around the jaw joint — often the source of headaches, facial soreness, and disrupted sleep. Botulinum toxin injected into the temporalis and masseter reduces the force these muscles generate, relieving pain and tension at the source.
Headache patterns vary, so care begins with an evaluation to map yours and target the right muscles. Treatment often mirrors TMJ care and can extend to the frontalis, glabella, or orbicularis oculi when your pattern calls for it.
Chronic muscle knots in the neck, shoulders, and upper back — particularly the trapezius — can refer pain to distant areas including the head and arms. Treatment combines botulinum toxin with PDO threads, injected directly into the trigger points to release the contracted muscle tissue and reduce pain at the source.
PDO threads are an absorbable medical-grade polymer placed directly into a tight, painful muscle knot to provide long-lasting relief. The thread acts like a continuous dry-needling treatment — mechanically disrupting the knot and increasing local blood flow to relax the muscle while the thread slowly dissolves over several weeks. The area is numbed prior to placement.
