These Terms of Use govern your use of bespokefamilymedicine.com (the “site”), operated by Bespoke Family Medicine, PLLC (“Bespoke,” “we,” “us,” or “our”). By using the site, you agree to these terms. If you do not agree, please do not use the site.
The content on this site is general information about our practice and about direct primary care. It is not medical advice, and it is not a substitute for evaluation, diagnosis, or treatment by a qualified clinician who knows your history.
Using this site, submitting an inquiry form, or booking an introductory call does not create a physician–patient relationship. That relationship begins only when you enroll as a member and we accept you into the practice under a signed membership agreement.
In an emergency, call 911 or go to the nearest emergency room. Do not use this site, our forms, or text messaging to report an emergency or an urgent medical need. Messages and form submissions are not monitored continuously.
Bespoke is a direct primary care practice. Membership is a direct agreement between you and the practice for the primary care services described in your membership agreement. It is not health insurance, not a health plan, and not a substitute for coverage. We recommend pairing membership with a high-deductible plan, catastrophic plan, or health-sharing arrangement for hospitalization, specialist care, and emergencies. Membership does not cover specialist visits, hospital care, emergency care, or medications except as expressly stated in your agreement.
Fees, included services, billing, and cancellation are governed by your membership agreement, which controls if anything on this site conflicts with it. Pricing and service descriptions published here may change. You may cancel your membership with 30 days’ written notice.
Our inquiry and scheduling forms are provided through Notive Health, our patient-communication and scheduling vendor, and are embedded from their systems. Your use of those forms is also subject to the vendor’s handling of the information you submit, as described in our Privacy Policy below. The site may also link to third-party sites we do not control and are not responsible for.
You agree not to misuse the site — including attempting to gain unauthorized access, interfering with its operation, scraping it at a scale that burdens it, or submitting false information or another person’s information without authorization.
The text, images, logos, and design on this site are owned by Bespoke Family Medicine, PLLC or used with permission, and may not be copied or reused without our written consent, except as permitted by law.
The site is provided “as is” and “as available,” without warranties of any kind, express or implied. We do not warrant that the site will be uninterrupted, error-free, or that information on it is complete or current. To the fullest extent permitted by law, Bespoke Family Medicine, PLLC is not liable for indirect, incidental, special, or consequential damages arising from your use of the site. Nothing here limits liability that cannot be limited under applicable law, and nothing here alters the standard of care owed to you as a patient.
These terms are governed by the laws of the State of Utah, without regard to its conflict-of-laws rules. Any dispute arising from the site will be brought in the state or federal courts located in Utah County, Utah.
We may update these terms. The effective date above shows when they were last updated. Continued use of the site after an update means you accept the revised terms.
This policy describes how we handle information collected through this website. Protected health information that we create or receive as your treating practice is handled under HIPAA and our Notice of Privacy Practices, which is provided to members at enrollment — that notice, not this policy, governs your medical record.
When you submit our inquiry form or book an introductory call, we collect what you enter: your name, email address, telephone number, date of birth if you choose to provide it, and the message or scheduling details you send. These forms are delivered through Notive Health on our behalf.
We use Google Analytics to understand how visitors use the site. When you consent, it collects anonymous usage data such as pages viewed, approximate region, referring site, and browser and device type, using cookies and similar technologies. We do not use it to identify you personally.
Analytics storage is denied by default. No analytics cookies are stored on your device unless you select “Accept” in the banner shown on your first visit, and your choice is remembered for later visits. Until you accept — and permanently, if you select “Decline” — any signal sent to Google Analytics is cookieless and is not used to identify you. You can also clear or block cookies in your browser settings at any time; the site works either way.
We use the information you submit to respond to your inquiry, schedule and confirm introductory calls, enroll you if you decide to join, operate and secure the site, and meet our legal and professional obligations. We do not use website inquiry information for advertising.
We do not sell, rent, or trade your personal information. We share it only with service providers who process it on our behalf under contract — currently Notive Health for forms, scheduling, and patient communication, and Google Analytics for anonymous site analytics — and where required by law or to protect the rights and safety of our patients, staff, or practice.
We use reasonable administrative, technical, and physical safeguards to protect the information we hold. No website or transmission method is completely secure, so please do not send detailed medical information through the website forms. We keep information as long as needed for the purposes described here and as required by law and our recordkeeping obligations.
This site is not directed to children, and we do not knowingly collect information from children through it. A parent or guardian may submit information about a minor when inquiring about care for that child.
You can ask us what information we hold about you from the website, ask us to correct it, or ask us to delete it, by contacting us using the details below. Requests about your medical record are handled under HIPAA and our Notice of Privacy Practices.

Bespoke Family Medicine, PLLC offers the Bespoke Family Medicine text messaging program. The program provides recurring clinic account and care-management messages to patients who choose to enroll. Messages may include appointment reminders and confirmations, pre-visit instructions, prescription-status notices, notices that information is available in the patient portal, billing or account notices, security alerts, and responses to patient-support requests.
The program is operated by Bespoke Family Medicine using technology provided by Notive Health. Bespoke Family Medicine is the sender of the messages.
By providing your consent, you are enrolling in the Bespoke Family Medicine text messaging program. Subscription is not required to create your account, receive medical care, or purchase goods or services from us. Consent applies only to this program and is not transferable.
You represent that you are the subscriber or customary user of the mobile number you enroll and that you are authorized to consent to messages at that number. Please tell us if your number changes or is reassigned.
Message frequency may vary. Message and data rates may apply. Your mobile carrier’s rates and terms govern charges to your device or service plan.
You may opt out at any time by texting STOP in reply to a program message. We may send one final message confirming your opt-out. After that confirmation, we will not send additional messages under this program unless you enroll again.

Wireless carriers are not liable for delayed or undelivered messages. Message delivery depends on your carrier, network availability, and device, and is not guaranteed.

We may update these terms or discontinue the program. The effective date above shows when they were last updated. Changes do not override an opt-out or authorize messages for which consent is required.

When you enroll in or communicate through the program, we may collect your mobile telephone number, your opt-in and opt-out choices, the date and method of consent, message content you send to us, and technical delivery information such as message status and timestamps.
We use this information to send and receive the clinic account, appointment, care-management, security, billing, and patient-support messages described in our SMS Terms & Conditions; respond to requests; honor messaging preferences; operate and secure the program; and meet legal and carrier requirements.
Information gathered in the SMS campaign will not be shared with third parties or affiliates for marketing purposes.
We do not sell, rent, or share mobile information with third parties or affiliates for their marketing or promotional purposes. Text messaging originator opt-in data and consent will not be sold, rented, transferred, or shared with third parties or affiliates for their own purposes.
Service providers that help us deliver and support the messaging program may process information solely on our behalf and may not use it for their own marketing. This includes Notive Health, our messaging provider, and the telecommunications carriers involved in delivering messages.
